5. Type of Ensemble Requested (If you note more than one, we will supply you with comparative rates):
a. Quartet (two violins, viola, and cello)
b. Trio (violin, viola, and cello)
c. Duo (two violins)
d. Duo (violin and cello)
e. Solo (violin or cello)
6. Please also provide a brief description of your event, and let us know if you have any specific questions or requests.
